Display State Health Plan Forms

ESS

Purpose:

Perform this procedure to access forms required by State Health Plan.

The State Health Plan Forms section is part of the 'My Benefits' Workset in ESS.  Employees can print the forms or save them their desktops.  Employees will need to submit the completed forms to Shared Services for processing.

The following State Health Plan Forms are available in ESS:

· Other Group Coverage Form

· Disability Form

· Foster Child Eligibility Form

· Medicare Eligibility Form

Note:  These instructions assume that you have already logged into the ESS/MSS portal. If you need assistance logging in, please refer to the Logging On to the BEACON Employee Portal instructions.

1. After logging on, the user will be defaulted to the BEACON Home Page.

2. Click the My Data (ESS)  tab.

ESS: Overview Page

Information:  The Overview page shows all the available Worksets in ESS.  A Workset is a grouping of web applications by functional area (e.g. Benefits, Time Management, etc.).

3. Access the My Benefits page by performing any of the steps below.

Select the Workset tab.  The Workset tabs are located below the Business Package tab (i.e. ESS, MSS, etc.).

 

Click the icon for the Workset, 'My Benefits.'

 

Click the main hyperlink for the Workset, 'My Benefits.'

 

Area: My Benefits

Information:  The Area page includes all the associated web applications for that Workset, the links to access the applications, and a short description of each application (if applicable).

4. To access the forms, scroll to the section called State Health Plan Forms.  The following forms are displayed in the section:

_ Other Group Coverage Form

_ Disability Form

_ Foster Child Eligibility Form

_ Medicare Eligibility Form

5. To launch a form, click its corresponding hyperlink.  For example, click.

6. The forms in ESS are launched in Adobe format as a PDF document.  Using Adobe functionality, perform one of the following:

 You want to print the form to your local printer.

 You want to save the form to your PC.

 You want to navigate to the previous or next page.

Click

 You want to decrease or increase the magnification of the entire page.

Click

 You want to fill the window with each page and scroll through pages continuously.

 You want to display one page at a time.

 You use a search/find function, such as type a word to find it in the displayed PDF document.

Information:  The State Health Plan Forms should be manually filled out by the employee.  Completed forms should be submitted to Shared Services for processing.
